The Leaving Care and After Care Team was established within the Local Authority on 1st April 2014, as part of a transformation of service provision to Children Looked After and Care Leavers. The Team consists of a Team Leader, Leaving Care Personal Advisors, a full time Supported Lodgings Coordinator and a full time Housing Advice Officer for Care Leavers. The Leaving Care and After Care Team on average work with 130 Care Leavers age 17-25.
The Leaving Care and After Care Team’s purpose is to improve the outcomes for young people leaving care.
The principles of the service delivery are:
- to delay young people leaving care until they are ready
- to improve planning for leaving care, including educational and housing plans
- to enhance support to young people, including preparation for independence
- to provide financial assistance to Care Leavers
The Leaving Care and After Care Team has a lead role in ensuring that young people leaving care, have the same opportunities as their peers and that they receive additional support to achieve these same ambitions.
